Introducing Bellevue Pharmacy's Patient Rewards Program!

We appreciate your business and in reward for your loyalty we have created a Patient Rewards Program where you can earn a reward twice yearly for your continuous use of our pharmacy.

Enrollment begins October 1, 2007.

Here's how it works:

Call our customer service team at 800.728.0288 and ask to be enrolled in the Patient Rewards Program. Bellevue Pharmacy will send out your personal wallet card on your next order to track your own refills. The card will include your eligibility period, which starts the day you enroll and ends 6-months afterward.

When you fill 5 prescriptions within your 6-month eligibility period you will earn a $15.00 reward for use on a future order! Once you have redeemed your reward, we will send out your new Patient Reward Card with your order and the new eligibility period will start at the end of the first eligibility period. No need to re-enroll!

The wallet card is for your convenience only--it does not need to be returned for redemption of reward.

When placing refills online and you want to start enrollment in the Patient Reward Program, simply type your request in the "Special Notes" area of the online form.

The following rules apply:

• Patient must fill* a prescription 5 times during eligibility period


• Patient is responsible for tracking number of prescription fills and redeeming reward


• Once reward requirements have been met, reward must be redeemed within 60 days after end of eligibility period


• Reward money can be used toward patient's next purchase


• Maximum of 2 eligibility periods within 12 month period


• Reward money must be used in entirety--one time use only, any left over amount cannot be carried over to future orders


• Prescription item does not account for days supply--a 30-day supply or a 90-day supply both count as one prescription refill

Example:

1. Jane Doe requests enrollment into Bellevue Pharmacy's Patient Rewards Program on October 1, 2007.


2. Bellevue Pharmacy sends Jane her personal Patient Rewards wallet card which contains the eligibility period of 10/1/07 through 4/1/08.


3. Jane records each refill she ordered and paid for on the personal wallet card.


4. Once the card is full (5 prescriptions), Jane has until 6/1/08 to use her reward. If Jane did not fill her fifth prescription until the last day of her personal eligibility period she still has plenty of time to use her reward!


5. Jane will call Bellevue Pharmacy to redeem her reward. Customer service will verify over the phone that all requirements have been met and the $15.00 reward will be applied to Jane's next order. Her new wallet card will be included in her next order.


6. Jane's next eligibility period starts at the end of her current eligibility period or on April 2, 2008.


7. The cycle keeps going and Jane keeps earning her rewards!
